In collaboration with the Xavier University of Louisiana Telehealth Center (XUTC), located in the College of Pharmacy, the XULA VOICE research group will be offering collaborative community grants to expand access to healthcare with a clinical pharmacist to address health disparities and increase vaccine uptake:

Health Care Services Provided:

• Medication Therapy Management (MTM): This involves a comprehensive review of a patient's medication profile to identify and resolve any medication-related issues, in collaboration with the patient and their healthcare provider.

• Chronic Care Management (CCM): XUTC offers detailed patient education and consultation, focusing particularly on diabetes and hypertension management.

• Diabetes Management Support for Rural Clinics: The center extends its services to rural clinics, providing diabetes management support in collaboration with other healthcare specialists.

• Work-Life Balance: The project will offer individual and group sessions to maintain a healthy and productive work-life balance.

• Vaccine Education: Individuals will be offered education on adult vaccinations, focusing on vaccines focusing on preventing respiratory illnesses such as influenza (flu), COVID-19, RSV and pneumococcal vaccine.

• Health Education Supplies: Enrolled participants will receive up to $600 for attending 7 Telehealth sessions, a tablet for Zoom(c) sessions and health education supplies including a Amazfit Band(c)

• Community partners will receive $8,500 for joining our research program.

We will be offering telehealth as an alternative platform to increase healthcare engagement for individuals from high-risk communities for respiratory infections (industry, healthcare, education, food industry, etc). The telehealth model will increase accessibility to pharmacy providers and provide counseling on key disease states: wellness, nutrition, diabetes, high blood pressure and vaccines.
